Left Main Coronary Artery Disease (LMCAD) is a blockage of 50 per cent or greater in the main coronary artery. It’s a serious condition associated with a high risk of mortality if left untreated.
The left main coronary artery is one of the heart’s most vital vessels; it supplies blood to the largest and most important areas of the heart muscle. LMCAD occurs when this artery becomes narrowed or blocked due to plaque build-up (atherosclerosis). Blockage in this artery is especially dangerous because it supplies the vast majority of our heart muscle, making timely treatment critical. Up to half of patients with LMCAD may die within a few years of diagnosis without appropriate intervention. Fortunately, with modern treatment, such as stents or bypass surgery, patients now have excellent outcomes.
What are the symptoms of LMCAD?
LMCAD doesn’t always cause noticeable symptoms at first, which is why regular medical assessments are vital. When symptoms do appear, they may include:
- chest pain or pressure (angina), especially during physical or emotional stress.
- heart burn/dyspepsia.
- shortness of breath.
- fatigue.
- dizziness or light-headedness.
- a heart attack.
If you notice any of these warning signs, especially if you’re over 50, it’s important to speak to your doctor to get a referral to Central Heart.
How is LMCAD diagnosed?
Central Heart is a specialist cardiology practice that’s equipped with a range of advanced diagnostic tools. Our team understands that timely consultations and testing are important, which is why we offer a private rapid access chest pain service, providing timely specialist assessment with coordinated stress echocardiography, non-invasive coronary angiography (CTCA), or definitive invasive coronary angiography for accurate diagnosis and management. We also strive to facilitate appointments for truly urgent cases within a week.
LMCAD is typically diagnosed using tests such as:
- Stress testing (stress echo)
- Coronary angiography
- ‘Virtual’ coronary angiography (Cardiac CT – CCTA)
Your cardiologist will recommend the most suitable test based on your symptoms and medical history.

Patient story: Chris
Chris had always been active and independent, but over six months, he started noticing some worrying changes. He felt tightness in his chest and became short of breath whenever he did anything physical, especially when walking up hills or mowing the lawn. The discomfort would ease if he stopped to rest, but it kept coming back. Determined to stay active, Chris began adjusting his routine to avoid triggering symptoms. But as his limitations became more apparent, so did his concern.
In March, his GP made an urgent referral to Associate Professor Tom Ford (A/Prof Ford). To avoid any delay, Chris was swiftly seen within four days by Dr. Kunwar Bhatia, an associate cardiologist at Central Heart. During this consultation, Dr. Bhatia performed an exercise stress test (stress echocardiogram), which revealed severely abnormal results, leading to the immediate decision to proceed with a coronary angiogram.
Within five days, Chris underwent the angiogram with A/Prof Ford, which confirmed severe left main coronary disease (blockage in the main heart artery). His case was discussed at the multidisciplinary Heart Team meeting — a collaborative model established by doctors Ford, Spina, and colleagues at Gosford Hospital, focused on delivering personalised, evidence-based care for complex coronary conditions such as LMCAD.
Following a thorough review, the team agreed that both stenting and bypass surgery were viable treatment options. After a detailed telehealth discussion, Chris and A/Prof Ford jointly decided that PCI was the preferred approach, recognising it as a less invasive option with similar long-term outcomes in terms of survival and risk of heart attack or death. While stenting carries a slightly higher chance of needing a repeat procedure in future, they agreed this was a manageable trade-off to avoid the risks and recovery associated with major bypass surgery. The procedure was performed via the right distal radial artery (right wrist) and guided by intravascular ultrasound (IVUS) to ensure precise stent placement and optimal results. A drug-eluting stent was inserted and further refined using advanced balloon technology. The outcome was excellent, with restored blood flow and only minor disease in other arteries manageable with medication.

Before (left) and after (right) technically straightforward stent procedure to the distal left main with 90% stenosis treated with a single stent from the wrist.
Chris recovered exceptionally well. He was discharged early the next morning, after a review with A/Prof Ford. They discussed the importance of continuing blood thinning medication for six months and enrolling in cardiac rehabilitation to support long-term recovery and heart health.
Chris’s story is a powerful reminder of how early action, expert care, and a collaborative heart team approach can lead to positive outcomes in LMCAD.
Why Heart Team discussions matter: Working together for the best patient outcome
For some patients with higher-risk heart conditions, we take extra steps to ensure you get the best possible care. One of those steps is a Heart Team meeting: a group discussion where experienced cardiologists, cardiac surgeons, and imaging experts come together (usually via video call) to review your test results and work out the best treatment plan for you. That might be surgery, a stent, or managing things with medication.
Not every case needs this kind of review. If it’s clear from the start that the best option is either surgery or medical treatment, we can usually move forward straightaway. But for many people, there’s more than one good option, and that’s where a team approach helps.
The Heart Team discussion ensures that:
- your care follows the latest expert guidelines.
- different specialists can weigh in with their unique knowledge.
- your preferences are respected, with clear explanations of the medical facts.
You don’t need to attend the meeting yourself, but we always take time afterwards to explain everything clearly, either through a telehealth follow-up or in-person. We’ll talk through the pros and cons of each option and help you make the right choice for your situation.
Ideally, this also means your procedure can be done by the most experienced person for that type of treatment, which can lead to better results and peace of mind. For example, choosing something like bypass surgery is a big decision. While it can work very well, it’s important to think about how it might affect your recovery, your lifestyle, and whether a less invasive option could work just as well in your case. Our goal is to give you the right information, so you can feel confident in the path you choose.
